Physician Referrals

How Our Referral Process Works:

1. Submit Your Referral
   Use the form below to refer your clients to us. Please include as much information as possible, including insurance details if available.
2. We’ll Handle the Scheduling
   After we receive your referral, one of our dedicated Patient Care Coordinators will reach out to your client to schedule their appointment at a convenient time.
3. Insurance Benefits Summary
   If insurance information is provided, we will review the coverage and supply a summary of Audiology benefits to your client.
4. Follow-Up Communication
   Once the appointment is completed, we will send a summary of the visit, including any recommended treatment plans and next steps, directly to your office.
